Prehospital Naloxone Administration as a Public Health Surveillance Tool: A Retrospective Validation Study

Background
Abuse or unintended overdose (OD) of opiates and heroin may result in prehospital and emergency department (ED) care. Prehospital naloxone use has been suggested as a surrogate marker of community opiate ODs. The study objective was to verify externally whether prehospital naloxone use is a surrogate marker of community opiate ODs by comparing Emergency Medical Services (EMS) naloxone administration records to an independent database of ED visits for opiate and heroin ODs in the same community.
MethodsA retrospective chart review of prehospital and ED data from July 2009 through June 2013 was conducted. Prehospital naloxone administration data obtained from the electronic medical records (EMRs) of a large private EMS provider serving a metropolitan area were considered a surrogate marker for suspected opiate OD. Comparison data were obtained from the regional trauma/psychiatric ED that receives the majority of the OD patients. The ED maintains a de-identified database of narcotic-related visits for surveillance of narcotic use in the metropolitan area. The ED database was queried for ODs associated with opiates or heroin. Cross-correlation analysis was used to test if prehospital naloxone administration was independent of ED visits for opiate/heroin ODs.
ResultsNaloxone was administered during 1,812 prehospital patient encounters, and 1,294 ED visits for opiate/heroin ODs were identified. The distribution of patients in the prehospital and ED datasets did not differ by gender, but it did differ by race and age. The frequency of naloxone administration by prehospital providers varied directly with the frequency of ED visits for opiate/heroin ODs. A monthly increase of two ED visits for opiate-related ODs was associated with an increase in one prehospital naloxone administration (cross-correlation coefficient [CCF]=0.44; P=.0021). A monthly increase of 100 ED visits for heroin-related ODs was associated with an increase in 94 prehospital naloxone administrations (CCF=0.46; P=.0012).
ConclusionsFrequency of naloxone administration by EMS providers in the prehospital setting varied directly with frequency of opiate/heroin OD-related ED visits. The data correlated both for short-term frequency and longer term trends of use. However, there was a marked difference in demographic data suggesting neither data source alone should be relied upon to determine which populations are at risk within the community.

Sufficient Catheter Length for Pneumothorax Needle Decompression: A Meta-Analysis

Introduction
Needle thoracostomy is the prehospital treatment for tension pneumothorax. Sufficient catheter length is necessary for procedural success. The authors of this study determined minimum catheter length needed for procedural success on a percentile basis.
MethodsA meta-analysis of existing studies was conducted. A Medline search was performed using the search terms: needle decompression, needle thoracentesis, chest decompression, pneumothorax decompression, needle thoracostomy, and tension pneumothorax. Studies were included if they published a sample size, mean chest wall thickness, and a standard deviation or confidence interval. A PubMed search was performed in a similar fashion. Sample size, mean chest wall thickness, and standard deviation were found or calculated for each study. Data were combined to create a pooled dataset. Normal distribution of data was assumed. Procedural success was defined as catheter length being equal to or greater than the chest wall thickness.
ResultsThe Medline and PubMed searches yielded 773 unique studies; all study abstracts were reviewed for possible inclusion. Eighteen papers were identified for full manuscript review. Thirteen studies met all inclusion criteria and were included in the analysis. Pooled sample statistics were: n=2,558; mean=4.19 cm; and SD=1.37 cm. Minimum catheter length needed for success at the 95th percentile for chest wall size was found to be 6.44 cm.
DiscussionA catheter of at least 6.44 cm in length would be required to ensure that 95% of the patients in this pooled sample would have penetration of the pleural space at the site of needle decompression, and therefore, a successful procedure. These findings represent Level III evidence.

Glioblastoma: Patterns of Recurrence and Efficacy of Salvage Treatments

Background:
It is controversial if distant recurrence of glioblastoma is more common after temozolomide (TMZ) concurrent with radiotherapy (RT). Optimal therapy for patients with recurrent disease after RT/TMZ is unclear. Our purpose was to evaluate recurrence patterns in glioblastoma and the effect of treatment at recurrence upon survival.
Methods:We performed a retrospective review of 67 patients with newly diagnosed glioblastoma treated with RT/TMZ between 2003-2007. Statistical analyses included Kaplan-Meier method for survival, and multivariate Cox proportional hazards model for the effect of salvage treatment on survival.
Results:58 patients (86.6%) recurred locally; 9 patients (13.4%) had a distant non-contiguous focus of new disease. Median survival(MS) was 17 months; median time-to-progression(TTP) 6.8 months. The local and distant groups had comparable prognostic factors. There was no difference in MS(p=0.35) or TTP(p=0.95) by location of recurrence. At relapse, 26 patients(38.8%) received continuous, dose-intense TMZ, 24(35.8%) other therapy(4.5% RT; 20.9% lomustine+/-procarbazine; 4.5% etoposide; 1.5% conventional TMZ; 4.5% TMZ then lomustine), and 17(25.4%) were untreated. Dose-intense TMZ was associated with prolonged MS compared to all other patients(21.5 months vs. 12.4 months, p=0.019, HR=3.86, 95%CI: 1.81-8.22) and similar to MS with other chemotherapy regimens(18.8 months, p=0.40, HR=1.30, 95% CI: 0.65-2.61).
Conclusion:The pattern of recurrence of glioblastoma treated with RT/TMZ was predominantly local. Second-line treatment with continuous dose-intense TMZ may prolong survival in patients with recurrent glioblastoma. Overall survival is similar to other conventional salvage regimens; however TMZ may be better tolerated. This study is limited by its retrospective nature and potential selection bias. Prospective controlled studies are needed.

The electrical heating of Ni/Al laminate foils allows interrogation of phenomena at heating rates as high as 10^12 K/s. In the 2011 Fall MRS meeting, we reported on emission spectra from rapidly heated Ni/Al laminates resolved temporally over 350 ns, which provided qualitative evidence of rapid and exothermic vapor phase mixing of Ni and Al in these experiments which we term electrical explosions. These results were significant, because thermal diffusion processes normally limit Ni/Al reactions to much slower energy release rates, potentially limiting their applications. Here we present further evidence of exothermic Ni/Al mixing, quantified by experimental velocity measurements of encapsulation material and interpreted by numerical calculations of energy partitioning into different processes. These calculations agreed well with experiments from different Al, Cu, and Ni samples, sputter-deposited and lithographically patterned into bow-tie bridge structures. Velocity measurements of up to 5 km/s for 11.5 μm thick parylene encapsulation layers were accurately predicted using a single, empirical fitting parameter which depended on the electrical circuit used. The calculations also agreed with encapsulation layers accelerated by electrically exploded Ni/Al laminates as long as an additional 1.2 kJ/g of energy was included in the model. This value is precisely the enthalpy of mixing between Ni and Al, and therefore quantifies the transduction of energy into encapsulation layer kinetic energy.

Paleoenvironment and taphonomy of the dinosaur-bearing Ischigualasto Formation (Upper Triassic, Argentina)

The oldest-known dinosaurs (Herrerasaurus, Pisanosaurus) occur within the Ischigualasto Formation. Recent work in the formation has brought to light significant new material, including the complete skeleton of a new primitive dinosaur. We sketch below the paleoenvironment and faunal succession during the range of these early dinosaurs, and review some of the taphonomic factors that shaped their fossil record.
The Ischigualasto Formation (Carnian?) is included within the Agua de la Peña Group, a series of continental Triassic deposits exposed in the Ischigualasto-Ville Union Basin of northwest Argentina. Ischigualasto sediments rest unconformably upon the carbonaceous fluvial/lacustrine Los Rastros Formation; this contact is characterized locally by marked angular discordance. The upper contact is gradational into red-beds of the Los Colorados Formation. Medium- to coarse-grained conglomeratic sandstones, siltstones, and silty mudstones dominate the section. Sand bodies are characterized by medium- to large-scale trough cross-stratification and broad lenticular/narrow sheet geometries, and are interpreted as deposits of shallow, low-sinuosity streams. Siltstones and mudstones show pervasive evidence of soil development, including root traces, nodular caliche horizons, and pedogenic slickensides. Deposits attributable to lacustrine/paludal sedimentation are scarce, and freshwater vertebrates and invertebrates are extremely rare. These data suggest an upland depositional setting on a low-relief alluvial plain with seasonal climate.
The Ischigualasto vertebrate fauna includes archosaurs, rhynchosaurs, traversodontid and carnivorous cynodonts, and temnospondyl amphibians. Rhynchosaurs dominate (relative specimen abundance) in the lower half of the section, but are absent from the upper half. Traversodontid cynodonts occur throughout the formation, but are much more abundant up-section. Archosaurs, carnivorous cynodonts, and particularly temnospondyls are rare throughout, with dinosaurs limited to the lower half. No major stratigraphic or sedimentologic changes occur up-section, and there is no evidence for significant shifts in physical or chemical taphonomic processes. Thus, trends in relative taxon abundance likely record a true biotic signal (e.g., local extinction, immigration) rather than a taphonomically-driven preservational bias.
Fossils are preserved as isolated carcasses or disarticulated elements, most often in fine-grained overbank facies. Bone beds and microsites are conspicuously absent. Temnospondyl remains were found within a local carbonaceous lens developed upon a sand body, suggesting autochthonous burial in an abandoned-channel setting. Isolated skulls, particularly those of the traversodontid Exaeretodon, are extremely common. Fifteen isolated crania of this cynodont were mapped in a single stratum with limited areal exposure. Abundant preservation of isolated therapsid crania has also been reported in the Beaufort Series (Permo-Triassic) of the Karoo Basin, South Africa (Smith, 1980). Post-disarticulation hydrodynamic sorting (enhanced by scavenging?) of an areally dispersed mass-mortality assemblage may explain this unusual occurrence.
